The content on this Site, including text, graphics, and information about conditions and services, is provided for general informational purposes only. It is not a substitute for professional medical advice, diagnosis, or treatment, and it does not create a provider–patient relationship. In an emergency

The Site is not for medical emergencies. If you are experiencing a medical or mental health emergency, call 911. Do not use this Site, our forms, or email to request urgent or emergency care.

Appointment requests & communications

Submitting a form or sending an email through the Site is a request to be contacted; it does not establish care or guarantee an appointment until confirmed by our team. Website forms and email are not secure channels; please do not transmit detailed health information through them. We'll arrange a secure way to communicate once you become a client.

Use of the Site

You agree to use the Site lawfully and not to:

  • Disrupt, damage, or attempt to gain unauthorized access to the Site or its systems
